Fortune Health Care Services, Inc.’s Community Development Services are dedicated to fostering the individual’s development and maintenance of skills related to community membership. This is achieved through active participation in community-based activities alongside individuals both with and without disabilities. Our Community Development Services can be flexibly delivered in various community settings, emphasizing activities that encourage heightened independence and inclusion. Utilizing a person-centered planning approach, we collaboratively define and pursue all desired outcomes and goals. Staff who care and support the participants include:
